We are committed to enhancing the lives and health outcomes of Medi-Cal Members in California. We work as a subcontracted agency in partnership with the California Department of Health Care Services (DHCS), specifically under the Community Supports initiative of the California Advancing & Innovating Medi-Cal (CALAIM) program. Our primary contracts are with Partnership HealthPlan of California and Independent Living Systems, LLC.

Our mission is to provide essential support services that significantly improve the quality of life for vulnerable populations. By aligning our services with the goals of CALAIM, we deliver innovative, community-centered supports that address the diverse needs of our members, promoting health equity and improved access to care.
Our Approach
We operate daily, providing services as needed, based on the hours or services authorized by our contracted entities. We are dedicated to serving following counties, ensuring that we are always available to support our members through direct care, community outreach, and administrative support.
